The most common reason why your Texas death certificate application will 100% get rejected by Texas Vital Records is if you’re a woman and you don’t put your MAIDEN last name and MAIDEN middle name down on the state-required Texas Designation To Obtain a Vital Record form.

Women will put their current last name down instead of their maiden last name or they’ll put their maiden last name down but as their middle name and not use their legally born middle name.
