Where do I email my
four ordering documents?
As of December 1st, 2022 you can only email us 3/4 of your ordering documents (ID photocopy, our order form, and a prepaid return FedEx or UPS shipping label) by attaching the documents and using our contact form located on our contact us page.
You must mail us the signed and notarized by a U.S notary Designation To Obtain a Vital Record form.
You can email us 3/4 of your ordering documents directly at the email address listed on all of our order forms (we don’t list it here or on the website in text form because of SPAM).
We highly recommend that you hire a U.S.-based e-notary, online, and virtual notary service to perform an electronic notarization via webcam on the Texas Designation To Obtain a Vital Record Form.
The electronic notary stamp should look similar to the example below and the e-notary public can be based in any of the fifty U.S states.
Then you can email us the electronically notarized Designation To Obtain a Vital Record form and you won’t have to mail us anything which will save you a lot of time, effort, and headaches.
